Because different conditions can produce similar symptoms, proper assessment is important before deciding on therapies, exercise or physical manipulation.
In classical Ayurveda, pain, stiffness, dryness and restriction of movement are commonly considered in relation to Vata dosha, although an Ayurvedic physician evaluates the individual's complete health picture before planning treatment.

Rather than automatically prescribing Kati Basti, massage or Panchakarma to everyone experiencing back pain, the physician considers the nature and duration of symptoms together with constitution, strength, digestion, lifestyle, previous treatment history and associated health concerns.
Following assessment, the physician can determine which therapies, medicines, dietary guidance, treatment intensity and program duration may be appropriate for the individual.
There is no single Ayurvedic therapy that is automatically the best choice for every type of back pain. Treatment procedures are selected and combined according to the physician's assessment.
Kati Basti is a traditional external Ayurvedic procedure in which warm medicated oil is retained over the lower-back region within a specially prepared boundary. It may be incorporated into a physician-guided care plan for selected individuals experiencing lower-back discomfort or stiffness.
The oil, duration and accompanying therapies are selected according to the individual's requirements. Kati Basti should therefore be understood as one procedure within a broader Ayurvedic treatment plan rather than a complete treatment by itself.
Abhyanga is a traditional Ayurvedic oil application and massage procedure performed with medicated oils selected according to individual requirements. Within a back-care program, it may be combined with other therapies based on the patient's condition and treatment stage.
Swedana refers to therapeutic sudation or heat-based procedures used in Ayurveda. Different forms may be selected depending on the individual's presentation and are often used together with other Ayurvedic therapies.
Njavarakizhi, also known as Shashtika Shali Pinda Sweda, is a traditional Kerala Ayurvedic procedure using medicated rice prepared and applied through cloth boluses. Herbal bolus treatments, oil applications and other Kerala therapies may also be considered according to the physician's assessment.
Panchakarma is a classical, physician-directed Ayurvedic process rather than a standard massage or detox package. Depending on the individual's constitution, strength, symptoms and overall health, specific Panchakarma procedures may be considered as part of a broader care program.
Not every person experiencing back pain requires complete Panchakarma treatment. The need for Panchakarma, individual procedures and duration should be decided following consultation.
